   Proposal Description:
   Understanding racial/ethnic disparities in Alzheimer's disease and related dementias (ADRD) is a public health priority according to the National Institute on Aging and the National Alzheimer's Project Act. ADRD is a major cause of mortality and disability, has a significant health impact on relatives, and its costs to the US healthcare system are higher than cancer or heart disease. By 2050, nearly 40 of older adults in the US will be racial and ethnic minorities. The risk of ADRD increases exponentially with age which means that racial and ethnic minorities will likely face a high burden of ADRD. However, few studies have compared longitudinally the risk of ADRD among different US racial and ethnic groups. The few that have done so have either focused mainly on African Americans vs non-Latino whites or relied on diagnoses from clinical records, or have not included MCI diagnosis or have focused on a specific region of the US or have not accounted for the competing risk of death, which is higher among certain racial groups. The present proposal aims to address these gaps by using data from the NACC UDS. The goals of the proposal are: 1) assess the risk of MCI and dementia between older adults in different racial and ethnic groups and 2) explore if modifiable risk and protective factors associated with MCI and dementia risk differ by race/ethnicity. The main hypotheses are: 1) African Americans and Caribbean Latinos will have a higher risk of MCI and dementia than non-Latino whites and  2) Asian Americans will have a lower MCI and dementia risk than non-Latino whites.    Analyses will include all cognitively normal individuals 55+ at baseline (inception of NACC UDS). The independent variable will be race/ethnicity and will include non-Latino white, African Americans, Asian Americans, Latinos (which will be probably divided into Caribbean Latinos, Mexican-Americans, and others depending on the sample size), American Indians and individuals of mixed race.
